Journey to Wellness is a grant funded year-long lifestyle change program that will give you the tools you need to become a healthier you. Designed for folks who live with pre-diabetes, have previously been diagnosed with Gestational Diabetes, or who score a 5 or higher the risk test for Prediabetes.

The Department seeks to strengthen individuals, families and our community with the services provided and is dedicated to providing effective and efficient programs.
The mission of ASI is to serve our community by empowering individuals who are 60 and better and their families to promote independence and advance community resilience.
Benefits in Action provides education, navigation, and application assistance for health insurance benefits and community and governmental benefits. This includes Medicare, Medicaid, Connect 4 Health Colorado, SNAP, LEAP, Social Security Disability, and more. Additionally, Benefits in Action provides a mental wellness program, a food delivery program, and provides pro-bono income tax preparation. All services are at no charge to the customer.
The Supplemental Nutrition Assistance Program, also known as SNAP or “Food Stamps”, provides nutritional food assistance for households who meet eligibility criteria.
The Broomfield Department of Human Services (DHS) provides assistance and support to Broomfield children, adults, and families. DHS services include food assistance (SNAP), health insurance (Medicaid), job search and employment services, and child and adult well-being.
