Aging and Disability Resources for Colorado (ADRC)
Aging and Disability Resources for Colorado (ADRC) is a visible and trusted source of information on the full range of long-term services and support options available to older adults, including:
- Assisting individuals in planning for their future long-term service and support needs
- Providing options counseling in a person-centered, interactive decision-support process
- Providing coordinated and streamlined access to information and assistance
- Providing personalized and consumer-friendly assistance to empower individuals to make informed decisions
The Aging and Disability Resources for Colorado provides options counseling for seniors or adults with disabilities and their caregivers to help provide guidance and insight on what programs and long term support services would best fit that individual’s needs. Option Counseling can be done over the phone, by appointment, or by home visit with one of ADRCs options counselors.

Services Include:
Home Connections
The goal of Home Connections is to help seniors remain safe and independent in their home by providing homemaker services, electronic monitoring, and case management including accessing services, home visits, and transitioning to higher levels of care..
Veteran’s Services
Veterans Directed Home and Community Based Services is a program for Veterans of any age who are determined by the VA to be at risk. With the help of a case manager, Veterans can direct their own care by managing flexible spending budget and personal care providers.
Supporting Our Seniors (SOS)
SOS is a transportation service for homebound seniors 60 and over. SOS provides seniors with one ride a week to doctor/ medical appointments or pharmacy pickups. This program is free and volunteer based. Apply here to get the process started.
Options Counseling
The ADRC can provide counseling for seniors and adults with disabilities on your options of programs and long term support services to suit your individual needs. Options Counseling can be done over the phone, by appointment or an in home visit.
Material Aid
Having the items you need to stay independent is important. We can help with financial assistance in getting walkers, dental care, and a wide array of other services and items. Please fill out our financial assistance application if you are interested.
Respite Program
Our Respite Program is for caregivers that are 60 and older caring for someone 60 and older. We can help you find a much needed break though day programs, providers coming into your home or a short stay at an assisted living facility.
Matter of Balance
MOB is an evidence based program designed to both decrease the fear of falling and reduce the likelihood of a fall by teaching useful exercises.
Stress Busters
This nine week, evidence based program focuses on teaching caregivers self-care in a small group setting. There are specific course tracks for caring for a loved one with dementia or with chronic illness. These programs are scheduled thought the year.
Long Term Support
We can help provide initial intake and functional assessment for those seeking long term care services through Medicaid waivers. Waivers include elderly, blind and disabled, children with a life limiting illness, community mental health support and brain injuries.
Transition Services
This targeted case management service helps individuals transition from a stay at an assisted living back into your home. We can help with life skills training, peer mentorship, transition set up, and getting meals delivered to your home.
Area Agency on Aging
Older individuals, their caregivers, or anyone concerned about the welfare of an older person can contact the Area Agency on Aging for information and referral to services and benefits in their community.
