JFS is a nonprofit human services organization that serves anyone in need – regardless of their circumstances or religious beliefs. JFS provides a variety of services to older adults as shown below:
Care Management:
Care managers work with clients and families to develop a comprehensive, individualized assessment & care plan to identify and meet all client needs. This may include: information, referral and assistance, connection to community services and benefits, ongoing coordination of in-home services, assistance with applications, support for various transitions of care, including future planning, & end-of-life issues, client focused advocacy.
Some free services are available for clients over the age of 60. Private pay options are available on a sliding fee scale.
Consultation:
Care Managers work with clients and families on a short-term basis to provide guidance on self managed care. This program is for clients and families who want to coordinate their own care, but need direction.
Private pay options are available for this service.
Counseling:
JFS provided specialized mental health counseling focused on the unique needs and challenges of older adults. Licensed counselors help older adults and/or their family members embrace transitions associated with aging, such as coping with losses, adjusting to physical disabilities, and/or conducting an overall life review. JFS is one of the only organizations that houses clinicians who can serve clients wherever it is most comfortable and convenient to them, be that a private residence, retirement community, assisted living or skilled nursing facility, or in the JFS office.
