- Don’t have dental insurance or Medicaid
- Income at or below 250% of poverty level

Single Entry Point/Options for Long-term Care
- Single Entry Point Agencies provide service case management, care planning, and referrals to other resources for Health First Colorado (Colorado’s Medicaid Program) members. The following target groups and ages covered include:
- Frail Elderly, Blind, Disabled (18 and above)
- Mental Health (18 & above)
- Brain Injured (16-64)
Services may include:
- Housekeeping
- Meal Preparation
- Personal Care
- Assisted Living Placement
- Electronic Monitors/Med
- Minders Respite Care
- Home Modifications
- Relative Personal Care

Aging and Disability Resources for Colorado (ADRC) is a visible and trusted source of information on the full range of long-term services and support options available to older adults, including:
• Assisting individuals in planning for their future long-term service and support needs
• Providing options counseling in a person-centered, interactive decision-support process
• Providing coordinated and streamlined access to information and assistance
• Providing personalized and consumer-friendly assistance to empower individuals to make informed decisions
