As an Aging Life Care Professional, I offer expert, independent guidance to help you make informed decisions, reduce stress, and ensure you or your loved one gets the care they need.
I start with a comprehensive assessment covering a care recipient’s limitations and diagnoses and what care they currently received, their goals and the goals of their family caregivers. Other areas assessed include support systems in place, the living environment, legal issues, and finances.
After the assessment, I create a plan that includes problem-solving and prioritizing needs with a longer-term view to the future and then meet with the family and/or individual, including virtual meetings to include family members who live elsewhere, to land on a final plan everyone can support. The plan can include finding, coordinating and managing in-home care, referral to financial and legal resources, recommendations for home modifications, on-going coaching for family caregivers, attending doctor appointments and facilitating communication between the doctor, client and family caregivers, and assisting in paying bills and managing finances. I am also available to serve as a guardian.
Particularly when there isn’t a family caregiver nearby, I can serve as the primary point of contact and be available for crisis intervention to help clients navigate through emergency departments and hospitalizations, rehabilitation stays and being their advocate to ensure that they are receiving the best care possible.
